Summary:
Assist the pharmacists in enrollment and management of patients in the Carolina Assessment of Medications Program (CAMP) by obtaining medication information and coordinating and prioritizing patient appointments with the pharmacist.
Currently this is a telecommuting role with occasional in-person meetings and trainings in the Research Triangle Park.
Our primary office is located in Morrisville, NC and we could return to traditional in-office work once the pandemic subsides.
Description of Job
Responsibilities:
Utilize internally developed systems and the electronic health record (EPIC) to document and track patient eligibility, patient communications and program enrollment Effectively and accurately communicate with patients in a way that is compassionate and exemplifies the principles of Carolina Care Schedule visits for the patient with the pharmacist for phone and tele-medicine based clinics.
Make appointment reminder calls to the patient Conduct a pre-visit assessment through telephone communication with the patient to determine patient's adherence to the prescribed medication regimen(s), verify pharmacy benefit coverage, and prioritize patient appointments with the pharmacist based on the results of the assessment.
Appropriately triage inquiries to relevant team members when additional expertise is required Complete patient medication histories through consultation with the patient, patient's physician, outside pharmacies and medical records.
Document medication histories in the medical record and communicate findings with the pharmacist Assist the patient by coordinating any required follow up visits with the provider, and assure the patient has the required medication supplies.
Notify the pharmacist or provider if medication refill authorization is needed.
Contact the patient's pharmacy benefit plan to complete prior authorization documentation Develop member and provider communication letters as needed Maintain program activities dashboards Assist in training of new employees and education of pharmacy students and residents Participate in and operationalize solutions for improvements in clinic utilization and outcomes.
Knowledge, Skills, and Abilities Preferences:
o Bilingual Spanish/English is highly desired o Ability to embrace and adapt to change quickly o Friendly, helpful, and focused on delivering excellent patient care in a team environment o Ability to communicate effectively with a culturally and educationally diverse population o Proficiency with Microsoft Office applications (particularly Outlook, Excel, Word, Visio), technology utilization, and Epic/electronic medical records programs o Exhibits evidence of being a self-motivated problem-solver, independent worker and a valuable team contributor with previous experience working remotely o Detail oriented, with a high level of organizational skills Other InformationOther information:
Education Requirements:
? HS diploma or equivalent.
Licensure/Certification Requirements:
? Registration with the NC Board of Pharmacy unless enrolled in a PharmD program.
? CPhT from PTCB or ExCPT from NHA unless enrolled in a PharmD program.
Professional Experience Requirements:
? Two years of pharmacy technician experience required.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
